Clinic providers to offer sports physicals at Philomath High School for area athletes

Providers from The Corvallis Clinic will be offering sports physicals at the Philomath High School Gym on Friday, May 31, from 2 to 6 p.m. Athletes from all area middle and high schools and youth sports programs can receive physicals. $25 fee will be donated to the Philomath Booster Club. Please arrive with the following forms filled out:

Physical Consent Form
OSAA Physical Examination Form
